In order to complete the purchase process, we require you to complete a detailed form regarding the person to be monitored, and we recommend you collect the information first. You will need to have the following information to hand:
- Personal Details
Name, Address, Telephone Number and Date of Birth - 2 local keyholders
Name, Address and Telephone Number - Medical details
Doctor's Name, Address, Telephone Number and any Medical Conditions such as epilepsy, diabetes etc.
The process may take some time to complete and although you can return to the site, or call us at any stage, please allow 15 minutes to complete this process.

You can save your details at any point during the purchase process by simply pressing the Save for later button.
